- Two (2) forms of valid Identification (Driver’s Permit /ID Card / Passport)
- Proof of Address: (Utility Bill) / Bank Statement / Letter from Landlord (2 Months)
- Proof of Income:
- Recent Job Letter (not more than two (2) month old)
- Pay slip (most recent: weekly / fortnightly / monthly) within (1) month
- If Self-Employed:
- Certified statement of additional income
- Certificate of Incorporation
